Who Can Benefit? 

• Older citizens with many common medical conditions
• Younger persons may also qualify
• Those with short-term problems like fractures or accidental injuries
• Handicapped or chronically ill persons with conditions such as stroke, arthritis, multiple sclerosis and various heart conditions
• Essentially any patient, who is under the care of a physician, resides in the MCH service area, may have their medical needs met in the home setting and is essentially homebound
